Collapses at One Specific Hour
What usually goes wrong after day three
Those three lines you wrote usually point to something much smaller than people expect. Somebody's hungry. There's a transition with nowhere to land. Something that should have happened an hour ago didn't happen, and now the whole hour is paying for it.
The mistake is trying to fix the entire afternoon when the actual problem is one specific trigger.
Stay narrow. Fix the trigger, not the hour.
And once you find that trigger, the fix usually belongs before the hour starts, not during it.

If everyone falls apart at 4:30 because they're hungry, the fix probably isn't something you do at 4:30. It's a 4:00 snack.
If the afternoon falls apart because nobody knows how to restart after lunch, the fix might be a five-minute transition that happens before you expect anyone to sit back down. The repair often belongs upstream from the chaos.
Why this is bigger than the one routine
You just repaired one window. But most homes have more than one: a morning window, an after-school window, a bedtime window, and each one can have a completely different trigger.
